Only became aware of this whilst reading the news today only applies to those of us that live in England

You have until June 23, 2021 to opt-out of your NHS data being shared with third-parties. Privacy campaigners have raised concerns about NHS Digital, which plans to make the medical histories of more than 55 million patients in England into a new database, including mental and sexual health data, criminal records and more sensitive information.

Here’s how to opt-out:

1. Download this simple form that we’re providing here:

Microsoft Word Version (https://docs.reclaimthenet.org/NHS-Data-Grab-Opt-Out-Form.docx)
Open Document (LibreOffice) Version (https://docs.reclaimthenet.org/NHS-Data-Grab-Opt-Out-Form.odt)
PDF Version (https://docs.reclaimthenet.org/NHS-Data-Grab-Opt-Out-Form.pdf)

2. Fill out the form.

3. Deliver the completed form to your registered GP. This can be by hand, by mail, or by email.
You can find the contact details for your registered GP here (https://www.nhs.uk/service-search/find-a-GP).

The form must be completed and delivered by June 23, 2021, to prevent your data from being uploaded.
